  • Specific Role
  • Liaise with Client and/or Carer/Guardian in relation to care plan activities, daily planning, recreational activities and Community Participation.
  • Assist Clients to develop and maintain positive healthy relationships with service providers, supports family and friends
  • Update databases and care plans as directed
  • Contribute to ensure  that services run as close to schedule as possible
  • Plan and document future and scheduled client activities as per care plan
  • Prepare care plans in accordance with Client goals and NDIS standards
  • Communicate with NDIS planners where necessary and relevant other Government and Non Government agencies relevant to client activity
  • Ensure that scheduled activities are risk assessed as necessary
  • Ensure that activity whereabouts are notified to supervisor
  • Ensure service promotional material is sufficient at all times and undertake photocopying as required
  • Contact Service Users and relay information as requested
  • Maintain contemporary knowledge of NDIS systems and practices
  • Maintain clear and professional boundaries with Clients
  • Attend appointments/other services with clinical case manager as required
  • Skills Essential

  • An understanding of issues relevant to people to include, but not limited to, who are transport disadvantaged including people who are frail aged, younger people, people with disabilities and their carers
  • An understanding of the Home Care Standards + National Standards for Disability Services + other funding agreements
  • Ability to work as a constructive Team Member
  • Customer service experience including the ability to engage with a diverse range of Service Users
  • Relevant qualifications or appropriate industry experience
  • Experience in consultation and identification of community needs
  • Ability to manage time and meet deadlines
  • Conflict resolution and negotiation skills
  • Ability to be innovative within budget restrictions
  • Ability to engage with diverse communities
  • Current drivers license and reliable vehicle
  • Verbal & communication skills
  • Policy & Procedural development and implementation
  • Ability to take direction
  • Ability to maintain confidentiality
  • Certificate 3 in Disability Services or equivalent with experience in Disability Service delivery
  • Demonstrated organisational, scheduling skills
  • Demonstrated ability to be self-motivated and show initiative
  • Knowledge of Work Health and Safety requirements with regard to Service User transport
  • Skills Desirable

  • Knowledge of the Port Stephens Local Government Area
  • LR liscence
