A fantastic permanent opportunity has become available for a well respected and reputable NFP organisation in Sydney. BRC are looking for experienced and qualified Aged Care & Disability Case Manager to work with complex clients conducting assessments, developing care plans and supporting clients to be independent in the community.
Main Duties:
- Complete assessments addressing the behavioral, clinical and psychosocial needs of the clients
- Develop and monitor the care plan, incorporating goals and co-ordinating services where necessary
- Provide advocacy and negotiate with relevant care services
- Assist with financial management of the clients budget and ensure they have a good understanding of their budget
- Provide clients and families with support, resources and information to enable the client to activiely particupate in their own development and self advocacy
- Follow all policies and procedures of the organisation
Essential Criteria:
- Degree in Nursing, Social Work or Similar
- Demonstrate good working knowledge and experience working with complex physical, medical, social and emotional needs within a range of adults
- Strong Casemanagement skills, ability to prioritise and complete comprehensive assessments in the community
- Good knowledge and understanding of the NDIS
- Excellent organisational and decision-making skills
- Brilliant written and oral communication skills
- NSW Driver’s License
