The Role:

 

VincentCare Victoria is seeking an appropriately-qualified person to take on the position of Case Manager (Key Worker) working within the Home Care Packages Program. This role creates partnerships with older people in the design of services and supports them to meet their needs and aspirations using a Consumer Directed Care approach. The Case Manager will demonstrate effective decision making practises, seeking to prevent poor experiences/outcomes for the client using their expertise to recover situations or plan for transitions.

Seniors Living comprises of a community based wellbeing centre in Blackburn, Assistance with Care and Housing Program in Gippsland and 160 Home Care Packages to help older people stay in their own homes for longer.

Selection Criteria: 

  • Qualifications in Case Management, Allied Health or Nursing with a minimum 3 years’ experience in community care.
  • Experience in working collaboratively with others to design, implement and evaluate solutions created for clients.
  • Experience in creating personalised plans with clients which are realistic, sustainable and owned by the client.
  • Demonstrated familiarity with Home Care Packages, having previously worked in the community home care environment, consumer directed care and a brokerage model of service.
  • Experience in assessment and engagement with people who are highly marginalised, socially isolated and difficult to engage, including actively identifying potential clients.
  • Computer literacy – Microsoft Office and other data bases.
  • Current Victorian Drivers Licence.
