Aftercare has an exciting opportunity for a Support Facilitator to join team based in Hurstville.
Using a recovery-oriented and person-centred approach, Support Facilitators will work to actively engage with clients and their networks to facilitate their improved wellbeing through better coordinated and integrated service delivery. They will work with clients to offer and seek solutions to complex issues in a resourceful manner in order to enhance individualised recovery processes. The ultimate objective of this role and initiative is to improve the system response to and outcomes for people with severe and persistent mental illness who have complex needs.
As part of Aftercare's Hurtsville team you will:
- Facilitating better coordination of clinical and other supports and services to deliver 'wrap around' care individually tailored to the person's needs;
- Strengthening partnerships and building better links between various clinical and community support organisations responsible for delivering services to the PIR target group;
- Improving referral pathways that facilitate access to the range of services and supports needed by the PIR target group; and
- Promoting a community based recover model to underpin all clinical and community support services delivered to people experiencing severe and persistent mental illness with complex needs.
- Supporting people to transition to the NDIS
- Providing Support Coordination to people with NDIS packages
Current Driver's Licence and use of personal car is an essential requirement for this role.
The Right Fit...
This role would suit someone with a mental health background. The successful candidate will have:
- Tertiary qualifications in a related health or welfare discipline are a requirement of this role.
- Ideally you will have experience working with people with severe and persistent mental health issues in the community and/or outreach setting.
- Demonstrated capacity to relate to clients and other stakeholders with dignity and respect
- Understanding of NDIS and the principles of duty of care, rights to privacy and confidentiality
- Good computer skills
