Wintringham is a specialist not-for-profit company which provides aged care services to those who are financially disadvantaged and have previously been homeless or are at risk of becoming homeless. Our regional team of Carers, Case Managers and Outreach Workers provide holistic care and support to clients who live independently in their own homes.
Wintringham is seeking a motivated and enthusiastic person with a commitment to social justice to join our multi-disciplinary community team based at the Hume Community Office (Shepparton) as a Fixed Term Full Time Case Manager for three months. This role will assist clients living in their own homes to maintain their independence and lifestyle of their choice by providing a high level of care. You will be helping address issues of social isolation for program clients and assist clients to maintain or improve their accommodation.
The responsibilities include undertaking initial assessments for new clients, developing and implementing Care Plans and supervising Care and Support Staff to ensure that acceptable standards of service delivery are provided. You will have excellent communication skills, a non-judgemental attitude and the ability to liaise with people from varying socio-economic backgrounds.
We are looking for candidates with the following;
- A qualification in Social Work, Allied Health or Welfare
- Experience in community Aged Care setting
- Current drivers licence with a reliable vehicle suitable for transporting clients
- Ability to cover annual leave for job share partner
