We are currently seeking an experienced Client Advisor to work with an established and new group of clients. Ideally you will have prior experience working within the aged care sector as a case manager.

To be successful in this role you must have:

• Tertiary qualification in the health and welfare discipline
• An understanding of the community care service and the needs of its clients
• Demonstrated experience in person-centred practises and complex case management
• A Current Working With Childrens Check and hold or be willing to undergo a National Police Check 
• A current and valid Drivers License 

Your responsibilities in this role will include:

• Providing specialised, comprehensive and coordinated case management and support to persons who are frail aged and/or with a disability
• Tailoring  packages of care to assist individuals and their families to live in the community
• Initial in home visits conducting assessments of the client
• Advocate and liaise with other service providers on behalf of your client
• Maintaining client budgets within set parameters, negotiating and providing brokered service provision for clients and quality improvement activities.
