BRC Social Care is currently seeking a Case Manager with Homelessness and AOD experience to work with families for a well respected Not For Profit Organisation in a great Inner City location for a 5 month fixed term contract. As a Case Manager you will be responsible for a complex caseload of clients and provide intensive support to homeless clients. Immediate start!

Main duties:

• Conduct referrals/assessments providing education and support to the client
• Mandatory report writing
• Support the families to access services and housing
• Work collaborateively with external and internal stakeholders such as accomodation providers to support clients needs
• Person centred plans

Ideal Candidate:

• Tertiary qualifications in a Health or related area
• Demonstrated case management experience and the ability to hit the ground running
• Understanding of trauma informed practise and issues relevant to people with mental illness and AOD issues
• Strong interpersonal skills and high level of energy, enthusiasm, reliability, resilience and flexibility
• To be immediately available for work
• First Aid Certificate
• Current Driver’s License
• Current NSW WWC check
