Fixed Term Full Time

We are currently looking for a motivated leader who is passionate about facilitating opportunities alongside people with disabilities to join our team as a Lead Practitioner - Community. There is one fixed term role available, ending in July 2019 with the possibility of extension.

The Role:

In this role you will connect people with a disability with community and support providers, helping them establish personally meaningful goals to effectively discover more to life than they ever felt possible. In addition you will be empowered to mentor, lead and manage the person we support’s direct support practitioners to ensure we are delivering on our commitment to be an advocate (through person centred planning) for the person we support.

About You:

You will have excellent leadership capabilities with the ability to manage multiple priorities and stakeholders, along with highly developed interpersonal communication skills to assist you in building rapport with the person we support, their family and their support providers. In addition you will have a strong sense of community with well-established experience working in a coordinator role within this sector previously.

Supplementing the above, you will hold at least a Certificate 3 in Community Services or related field (or equivalent professional experience), and have intermediate systems experience (including MS Office and Care Management Systems) and a current Qld Driver Licence. You will also hold a current Working with Children Check (Blue Card) and a positive notice for working with people with a disability (Yellow Card), or have the ability to apply for these.
