As part of the Complex Client Services Team, Targeted Care Packages (TCPs) aim to achieve better outcomes for children and young people in out-of-home care. TCPs provide an opportunity to find alternative placements tailored to the needs of the young person. This will include speciailist therapeutic intervention & support services that wrap around the young person to support them to achieve and thrive with hope for their future.

The Coordinator will engage with young people, their Care Teams, families, communities and others towards building a sustainable and secure placement for the young person. The Coordinator will lead the relationship with the funder with respect to TCP’s, will develop, monitor and ensure quality reporting on TCPs, as well as supervise and support Key Workers.

The successful applicant will have a minimum of 3 years working within the welfare sector, knowledge and demonstrable experience in delivering innovative and effective approaches for young people (specifically under 12) with complex needs. Knowledge of the Out of Home Care system and Child Protection statutory requirements as well as demonstrated ability to achieve positive outcomes for young people through a strengths-based, outcomes-focused case management approach, will be required in this role.

A relevant tertiary qualifications in in Youth Work, Social Work, Community Services or equivalent and/or experience in the field will be vital for this role.
