About the Company
An organisation with long standing service history and is growing from strength to strength. It provides the care for clients with Mental Health, Traumatic Brain Injuries and/or dual diagnoses. It is committed to providing efficacious support to their clients to ensure maximum independence and healthcare. It provides services in a residential setting, in the community and with corrections.
About the Role
As a Clinical Coordinator, you will coordinate service delivery of you clients with long term disabilities. You will develop therapeutic relationships with clients, whanau and liaise with all members of the health care team.
Duties
• Develop and implement client care plans with a multidisciplinary team approach
• Complete continuous needs based assessments
• Ensure rehabilitation plans focus on community reintegration
• Take on Key Worker role for client load
• Liaise with internal and external stakeholders
Skills & Experience
• Registered Health professional with a current APC or registered with a professional body
• At least 3 years experience in Mental Health and Traumatic Brain Injury essential
• Community rehabilitation skills are preferred
• IT savvy
• Full clean driver’s licence
