As a member of an extended care team in Taranaki, you will be tasked with providing assessments and appropriate treatment to patients with diabetes high risk feet. You will provide education, coordination of care, and promotion of independence and self-management.
To be successful in this role you will have:
- registration with the New Zealand Podiatrists Board
- a passion for improving client health through evidence based best practice
- a keen ability to develop and maintain key relationships within a multi-disciplinary team, external agencies and supports
- experience working with whanau/person centred approach to care delivery
- an understanding of diabetes care
- an intermediate to high level of computer literacy
- a full, current and clean NZ driver licence

This is a full time role however part time hours may be considered for the right candidate.
