Care Coordinator – 2 x Permanent Full Time Positions

Then you're just the person we're looking for to join our care coordination team based in Petone, Lower Hutt.
Responsibilities include:
- Making sure the right people are there at the right time to provide support for our clients
- Managing client queries, escalations and incidents
- Setting up and maintaining rosters for our support workers
- Being the face of Access for our clients, funders and partners
Ideally you'll be:
- Community minded and people driven
- Optimistic
- Tolerant and reliable
- Friendly
You'll also have:
- Outstanding communication skills
- The ability to effectively manage conflict
- Awesome time management
- Excellent negotiation skills
- A passion for delivering excellent outcomes for our clients and their families
