The Bay of Plenty District Health Board is seeking to recruit an Administrator to support the management and operation of a Community Care Co-ordination Demonstration Site. This is an exciting opportunity be part of a team managing the successful daily operations of a Community Care Co-ordination service. We are seeking full time administration support person to operate within a rostered  7 day service - fixed term until March 2019.


Community Care Coordination is established as a demonstration site for 12 months until 3 March 2019, with a view that an evaluation of the service is completed and presented ahead of this tenure. This is a seven day service with a team consisting of administrative staff, registered nurses and an operations manager.

 
The aim of the demonstration site is to:
  1. have a single place for referral and prioritisation and deployment of referrals to community nursing services which is the first step towards integrating community nursing across the Bay.
  2. provide a place for referrers and patients alike to access care information and a platform for access to wider community care coordination.

The administration support team is collectively responsible for progressing and responding to requests for community nursing care and customer service support for Community Care Coordination demonstration site. Administrators will be the first and main point of contact for enquiries regarding access to community nursing care, and will provide support for a team of Triage Nurses and the Operations Manager.

 

The successful applicants will be able to work effectively within a complex service environment and be able to establish and maintain collaborative relationships within the Community Care Coordination service and with key community agencies, healthcare providers, hospital services, general practice and stakeholders within health and across the community.

 
Ideally you will have:
  • Excellent customer service skills with a strong focus on patient and family/whanau centred care;
  • A minimum of two years office administration within a health environment with system and process administration focus;
  • Advanced computer skills and excellent phone manner;
