The role

Your responsibilities will include:

• Providing initial assessment and ongoing treatment to elderly clients and clients with disabilities
• Producing thorough and accurate documentation and completing funding application reports (if required)
• Liaising with service providers and within the team to ensure effective communication
• Based around Parramatta and Sydney's western suburbs
• 2-3 days a week

Essential criteria

• Experience working in the community to provide Home-Based Allied Health Service is very desirable
• 2 - 3 years experience in an age care setting
• Ability to work effectively and with empathy
• Understand the complex needs of elderly clients and clients with physical disability
• Current AHPRA Registration, police check and Australian driver's license
