An exciting opportunity to be part of the development and trial of a new service. As the Case Manager, you will be responsible for working with clients, their families and relevant others to take a holistic approach to the social and health needs of clients with a goal of reducing the number of unplanned admissions to hospital by successfully integrating the care needs of clients.
To be successful in this role, candidates will be able to demonstrate the following experience, skills and attributes:
- 5+ years’ experience in a post graduate clinical experience in field of expertise
- Relevant and current Registered Nurse qualification
- A proven ability to develop effective support plans which addresses client need within budget constraints
- Demonstrated understanding of the challenges that colleagues in the health and community sectors face and the ability to work effectively with them.
- Relevant experience in providing care for complex health care needs in the community.
