Midland Nursing Home provides 24-hour individualised care and lifestyle support for 110 consumers.

About the Role

Your role would be autonomous and each day would be different. You'll have the opportunity to work individually, within a multi-disciplinary team, and work with great people. You will apply your clinical reasoning skills, in order to achieve great outcomes for our consumers and their families working towards Midland Nursing Homes vision and in accordance with our core values.

Objectives of the Role:

  • Ensure that the consumer gets safe and effective services and supports for daily living, that meet the consumer’s needs, goals and preferences and optimise their independence, health, wellbeing and quality of life.
  • Ensure that services and supports for daily living promote each consumer’s emotional, spiritual and psychological wellbeing.
  • Ensure that services and supports for daily living assist each consumer to:
    • participate in their community within and outside the service;
    • have social and personal relationships; and
    • do the things of interest to them.
  • Ensure that information about the consumer’s condition, needs and preferences is communicated within the organisation, and with others where responsibility for care is shared.
  • Ensure that timely and appropriate referrals to other providers, organisations and individuals.
  • Where equipment is provided, ensure that it is safe, suitable, clean and well maintained.
  • Ensure that each consumer receives safe and effective occupational therapy services that are; based on best practice, tailored to their needs and optimises their health and wellbeing.
  • Ensure that high-impact or high-prevalence risks associated with the care of each consumer are effectively managed including:
    • minimising restrictive practice;
    • supporting people living with dementia and other chronic medical and mental health conditions;
    • minimising risk and harm from falls;
    • preventing and managing pressure injuries        and wounds;
    • preventing, identifying and managing delirium; and
    • supporting consumers to live without pain.
  • Ensure the needs, goals and preferences of consumers nearing the end of life are recognised and addressed, their comfort maximised and their dignity preserved.
  • Deterioration or change of a consumer’s mental health, cognitive or physical function, capacity or condition is recognised and responded to in a timely manner.

Key Duties:

  • Conduct all Occupational Therapy services commensurate with the objectives of this role.
  • Formulate comprehensive assessments and complete reviews that will identify consumer’s functional capacity, strengths and lifestyle needs, preferences and goals using a consumer-directed approach. 
  • Devise, review and evaluate individualised consumer therapy plans and programs to optimise consumer’s independence, health, safety, wellbeing and quality of life.
  • Provide individualised therapy services, including equipment prescription and modification, enabling elderly consumers to maintain functional capacity, manage deterioration, maximise quality of life and re-engage with the community via a variety of programs and activities.
  • Contribute to the development of holistic, consumer-directed and person-centred care plans in partnership with consumers and their representatives and the Midland Nursing Home team.
  • Develop, manage and evaluate a dedicated Group Therapy Activity Program.
  • Produce comprehensive analysis of performance of the overall activity program on a monthly basis.
  • Actively contribute and participate in the development of the quarterly newsletter.
  • Coordinate, host and manage the outputs of the consumer and representatives meetings held bi-monthly.
  • Contribute to Midland Nursing Home’s organisational governance requirements, processes and arrangements in order to assist with implementation, risk management and continuous improvement.
  • Support the established internal auditing program at Midland Nursing Home by conducting regular audits as directed by the Quality Committee.
  • Work efficiently and effectively within a Multidisciplinary Team (MDT).
  • Oversee, support and drive the performance of therapy assistants / volunteers / students and other visitors to site from external agencies.
  • Conduct regular performance reviews of all Therapy Assistants and report all outcomes to the HR Officer and Executive Manager.
  • Allocate, account for and manage all equipment utilised for Occupational Therapy services.
  • Maintain current knowledge of contemporary Occupational Therapy techniques and healthcare issues, based on best practice and ensure all clinical practice is in accordance with the relevant legislative requirements, inclusive of the anticipated Aged Care Quality Standards, Occupational Therapy Code of Ethics, Code of Conduct and available evidence.
  • Actively participate in consumer reviews, case discussions and meet reporting time frames.

Essential criteria:

  • Current AHPRA Registration.
  • A valid police clearance.
  • Demonstrate genuine passion for aged care
  • A willingness to learn, develop and adapt to changes, to drive and implement continuous improvements.
  • Demonstrate excellent time management skills and the ability to problem solve and critically think.
  • Positive work ethic and demonstrated ‘can-do’ attitude.
  • Ability to work autonomously and within a team.
  • Strong computer skills with advanced MS Office skills.
  • Well-developed strong and clear communication skills.
  • Excellent attention to detail.

  • Knowledge of Lee care electronic health care systems.
  • Knowledge of the anticipated Aged Care Quality Standards.
