About the Role
We are seeking a suitably qualified Clinical Case Manager to join the #iConnect team based in our Hobart office on a permanent, full time basis.  As Clinical Case Manager, you will be responsible for clinical case management and direct provision of therapeutic psycho-social support, and development and implementation of care plans. 

Key Responsibilities
  • Work therapeutically 1:1 with young people and families with complex needs in a community outreach capacity, engaging in evidenced based psychological interventions and therapeutic psycho-social support.
  • Monitor and review clients' progress through the service, and against the care plan, and provide up to date progress reports.  
  • Ensure that client management systems are up to date with all relevant client and service information.
  • Effectively plan and organise own work effort and regularly provide feedback to the manager on work progress.
  • Work collaboratively with the client, family and the care team to plan for, and develop the skills to exit or transition from the service, including referral to other services where appropriate. 
  • Contribute to the evaluation and assessment of the service programs and processes
  Skills & Experience
  • A tertiary degree in Psychology, Social Work, Occupational Therapy, Mental Health Nursing or other relevant qualification  
  • Demonstrated experience engaging in  work with clients and families 1:1 with complex needs using a range of evidence based clinical interventions. 
  • Demonstrated capacity to work both as part of a multi-disciplinary team, and autonomously in community outreach.
  • Demonstrated experience in managing competing priorities and to deliver outcomes within agreed timeframes and quality standards
  • Highly developed analytical skills and demonstrated experience in carrying out clinical assessments, developing care plans, and interventions with clients, using a range of evidence based psychological interventions.
  • The ability to work collaboratively with agencies that support people with complex and challenging needs such as with mental health services, GPs, Psychiatrists, or other statutory or community based services.
Successful candidates will be required to clear probity checks including National Criminal History Record Check and hold Tasmanian Working With Vulnerable People Registration.
