About SEMPHN

South Eastern Melbourne Primary Health Network (SEMPHN) has the key objectives of:

  • Increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and
  • Improving coordination of care to ensure patients receive the right care in the right place at the right time.

We achieve these objectives by working directly with general practitioners, other primary health care providers, secondary care providers and hospitals to ensure improved outcomes for patients.

The six key priorities of SEMPHN are mental health, Aboriginal and Torres Strait Islander health, population health, health workforce, eHealth and aged care.

SEMPHN stretches from St Kilda to Sorrento to Bunyip, including the major population hubs of Clayton, Dandenong, Moorabbin, Caulfield, Cranbourne, Frankston and Pakenham.

About the role

The Suicide Prevention Lead will work with a functional level of autonomy to:

  • facilitate networks and strategic engagement;
  • conceptualise systems and diagnose trends, obstacles and opportunities in the internal and external environment;
  • manage and lead projects;
  • make local program related decisions;
  • establish and maintain key working relationships; and
  • manage the commissioning of suicide prevention interventions that will focus on capacity building and enhancing system effectiveness.

The Suicide Prevention Lead reports to the Manager Service Development (Youth Mental Health and Suicide Prevention) and liaise closely with local services and established networks to support the implementation of all aspects of the suicide prevention program.  Additionally, the Suicide Prevention Lead will work with internal teams such as Service Innovation, Contracts, System Outcomes, Provider Support and other external stakeholders.  This also includes working collaboratively with our stakeholders and consumers to support the commissioning of solutions in suicide prevention.

Qualifications, Skills and experience

To be successful in this fantastic opportunity you must have the following qualifications, skills and experience:

  • Relevant tertiary qualifications or equivalent relevant experience.
  • Demonstrated experience in health and community service programs.
  • Demonstrated knowledge and experience of one or more of the following; mental health service delivery, suicide prevention, community development, care coordination, project management, change management, clinical redesign.
  • Experience working with a range of health care providers including General Practices, nurses, and Allied Health Providers including consumers and their natural supports.
  • Demonstrated experience managing projects.
  • Proven ability to identify innovative solutions and influence outcomes.
  • Strong analytical and problem solving skills with the ability to analyse issues and provide advice upon which decisions can be based.
  • Demonstrated experience in influencing, negotiating and engaging positively with a diverse range of stakeholders to achieve improved outcomes.
  • Stakeholder relationship management skills to support effective systems change with improved health outcomes.
  • Strong Microsoft Office skills.

Travel internal and external to the region will be required.
