Part Time 30.4 hours per week
12 month fixed term contract.

The Wathaurong Aboriginal Health Service is a fast growing and innovative health service that aims to provide the local Aboriginal community with culturally appropriate, high quality care. The following position is now available:-

The successful applicant will be responsible for the Care Coordination and individual support for community-based recovery and rehabilitation services supporting people living with mental illness and psychiatric disability to improve their health, live independently and pursue a life based on their own strengths, values and goals.  You will use a Collaborative Recovery Model (CRM) to support people through our services by identifying their personal strengths and values, to set goals and then support them to make progress towards achieving them.

This is an Aboriginal and Torres Strait Islander identified position, this action constitutes a special measure under Section 12 of the Equal
Opportunity Act 2010.

If you have experience in related areas of work and a solid understanding of the issues confronting Aboriginal communities then we want to hear from you.
