Pathways to Community Living Initiative - Senior Social Worker You’ll be working as part of patient centred, recovery focussed multi-disciplinary team alongside and integrating with other clinical teams on the Macquarie Hospital Campus. As part of this initiative, supporting the successful transition to other support and accommodation will see the role and support move with consumers into the community to support holistic clinical and non-clinical planning, partnerships and ongoing review and assessments. What you'll be doing You will be playing a central role in PCLI service delivery ensuring timely, flexible, high quality recovery orientated care is provided. In addition to liaising closely with consumers and their families and/or carers you’ll be working closely with other functions and teams to ensure seamless integration and co-ordination of day to day services and provide advice and support as needed. The position carries a clinical workload that assists with discharge planning and placement to a range of community supports and accommodation. The role actively supports transition into community setting and provides follow up and assessment in partnership with community based clinical and non-clinical services. Employees must abide by the NSW Health Code of Conduct. Criminal record checks will be undertaken on successful applicants and applicants for positions designated as Category A will be required to obtain required vaccinations prior to commencing employment. Candidates will need to meet the following criteria: Consistently demonstrates behaviours that reinforce the CORE Values of our organisation; Collaboration, Openness, Respect and Empowerment. Demonstrate these behaviours with all stakeholders; colleagues, direct reports, as well as our patients and consumers, and those that care for them. Tertiary qualifications in Social Work providing eligibility for membership of the Australian Association of Social Workers with extensive post graduate social work clinical practice. Demonstrated skills and knowledge in assessment, counselling and therapy interventions, case management, advocacy and discharge planning. Demonstrated understanding of the impact and management of Trauma, Sudden death, Bereavement, Domestic Violence, Child Protection, Guardianship. Demonstrated experience in effective staff management, supervision and the development of staff at all levels. Demonstrated experience in service development and quality improvement. Demonstrated excellent written and verbal communication skills, time management and computer literacy. Need more information?
