For more information visit Northern NSW Local Health District What you'll be doing The Patient Flow Manager (PFM)/ Discharge Planner represents the facility’s leadership team and is responsible for patient flow, bed management and discharge planning. The PFM is part of the management team supporting clinical staff in the planning, delivery and evaluation of integrated clinical care; ensuring clinical care for the patient within the service is safe, well managed and of the highest quality and linking effectively to the external services based on individual needs. Clinician engagement and leadership has been a prominent feature of the public health system for a number of years, where clinician led clinical standards, safety and quality improvements have become a major focus. The LHD is committed to the strengthening and ongoing involvement and clinician engagement of staff that provide direct patient care in the planning, delivery, improvement and evaluation of health services. Current registration with Nursing & Midwifery Board of Australia as a Registered Nurse (division 1), relevant recent experience in clinical leadership and management and/or post graduate management qualification Contemporary knowledge of evidence-based Patient Flow practices and experience in designing, implementing and evaluating initiatives that support Patient Flow, inclusive of effective principals for Discharge Planning. Demonstrated knowledge and understanding of the challenges associated with the management of nursing/midwifery resources within a dynamic environment and the proven ability to prioritise and make sound decisions that support the delivery of effective and efficient health care. Demonstrated knowledge and understanding of contemporary issues relating to nursing/ midwifery professional practice and the proven ability to effectively lead a nursing/ midwifery team within a complex multidisciplinary team environment. Highly developed and proven communication skills that include coaching, conflict resolution, mediation, delegation and negotiation skills and the ability to write reports of a high standard. Demonstrated computer literacy and ability to use a variety of computer applications (e.g. word processing, excel, databases and Intranet) Understanding of and commitment to the effective and meaningful engagement of clinicians in the decision making process that impacts on positive patient outcomes Need more information?
