About the Department
Prague House is a 45 bed low level facility catering to the needs of people who have been homeless, often with a mental health diagnosis, who are looked after by a skilled and compassionate multidisciplinary team.
About the Role
An opportunity currently exists for a Grade 2 Personal Care Worker to join the Prague House Team on a part time permanent basis. You will be responsible for providing personal care services to the residents of Prague House that is in line with organisational policies and procedures.
Duties
• Demonstrate a Person Care Approach in all aspects of care practice
• Liaise with residents, family members, nursing and medical staff as required
• Participate in care plan evaluation and team meetings
• Maintain documentation to meet ACFI Guidelines and Accreditation Standards
Qualifications, Skills and Experience
• Certificate 3 or higher in Aged Care
• Understanding and compassion of aged care residents
• Demonstrated ability to work as a member of a multidisciplinary team
• Highly developed communication skills
• Previous experience in a hospital and/or residential facility would be highly desirable
