About us:

One Door Mental Health is committed to improving the circumstances and welfare of people with a serious mental illness, their relatives and carers, and professionals working in the area. To achieve its aims One Door provides advocacy, mental health services, education and service development for people living with a mental illness and their families.

The Role:

Hospital to Home is a program designed to support people leaving hospital and returning to their own homes. We aim to provide one to one support to assist individuals to with the transition to home, make community connections and re-establish their lifestyle routines.

The Hospital to Home Coordinator will supervise and support a team of Peer Workers to deliver the Hospital to Home program in Western Sydney. You will work closely with the Local Health District teams at designated hospitals to establish the Hospital to Home Program, attend discharge planning sessions, provide education on the program and establish partnership arrangements. You will be responsible for recruiting, training and supporting Hospital to Home Peer Workers who will have had lived experience themselves of mental illness and the desire and ability to use that lived experience to inform their work as hospital to home facilitators. You will participate in evaluation strategy for the program and provide regular reports to the Regional Manager and funding bodies.

What you can bring to the role:

To succeed in this role you will need and a current and valid driver's licence and access to a fully insured vehicle (work related travel allowances will be paid). Previous experience in managing a team, the delivery of mental health services, knowledge of the NSW mental health system and an understanding of the recovery based approach will be key to being successful in this role. Tertiary qualification/s in a relevant discipline is vital. Previous experience working with Peer Workers will be advantageous.
