Capital Health Network and COORDINARE (primary health care networks for the ACT and Southern NSW) are seeking to recruit a person who has high level communication and influencing skills to fill the role of Program Manager for the HealthPathways program. This role will provide high level efficient and effective program management to the HealthPathways initiative including support to the two HealthPathways Coordinators and the clinical teams and contributing to the efficient running of the HealthPathways initiative.
HealthPathways is an online health information portal used at the point of care by GPs, specialists, nurses and allied health practitioners to assist with timely assessment, management and referral of patients to available services. It is underpinned by evidence based practice.
