Austin Health is the major provider of tertiary health services, health professional education and research across 3 campuses in the northeast of Melbourne and state-wide. Austin Health is world-renowned for its research and specialist work in cancer, liver transplantation, spinal cord injuries, neurology, endocrinology, mental health and rehabilitation.
An exciting opportunity currently exists for an enthusiastic and highly experienced Case Manager to join our Aged Care Community Programs in a Fixed Term Part Time role with our Transition Care & Restorative Care Programs with possibility of ongoing casual work to follow
To be successful in the role you will have:
- A recognised tertiary qualification in a health related discipline
- Current registration with Australian Health Practitioner Regulation Agency (AHPRA) or eligibility for registration with relevant body
- Knowledge of relevant aged care community and residential care resources
- Well developed care planning and assessment skills
- Extensive experience in service provision to elderly people with complex needs to ensure effective treatment and discharge planning
- Effective interpersonal, organisational, problem solving and decision-making skills
- A commitment to working within a multidisciplinary team
- Competence with Microsoft Word computer programs
- An understanding of the Australian Government Department of Health and Ageing Transition Care Program Guidelines
- An understanding of the interface between the acute/subacute and aged care sectors.
- A current Victorian Driver’s Licence
The position is located at Heidelberg Repatriation Hospital with some off site work at a care facility in Reservoir and within the community setting for our home based clients.
