About us

Central and Eastern Sydney PHN is a Not-For-Profit Primary Health Care Organisation covering Sydney Local Health and South Eastern Sydney Local Health District regions.

Our aim is to increase the efficiency and effectiveness of primary health care services for patients particularly those at risk of poor health outcomes; and improve coordination of care.

About the Role

The successful applicant will be responsible for the delivery of digital health initiatives to the health professionals in CESPHN catchment and providing ongoing support. These initiatives include My Health Record, secure messaging, data quality improvement and other digital health initiatives as required.

Key Responsibilities  

  • Undertake work on digital health and quality improvement initiatives such as My Health Record, digital health Practice Incentive Payments, and data quality improvement (QI) activities.
  • Provide client visits, meetings and telephone support to medical specialists, general practices, allied health professionals, universities and non-government organisations on digital health and QI related activities.
  • Work with team members to support general practices around person centred approaches taking into account capacity and capability requirements and tailoring support to practices.
  • Develop and implement targeted, appropriate and sustainable educational activities to address digital health.

Essential Criteria

  • Tertiary qualifications in an IT or health informatics field and / or relevant experience in digital health;
  • Strong IT skills and knowledge in the use of data bases, Microsoft Office suite, data security and clinical software used by health care providers;
  • Demonstrated ability to work autonomously, set project goals, prioritise tasks and troubleshoot in order to achieve key objectives within designated timeframes;
  • Ability to build, develop and sustain positive partnerships and productive working relationships with all levels of stakeholders, including individuals from diverse cultural backgrounds;
  • Experience providing training and support to health care professionals and community members;
  • Access to a motor vehicle and possession of a current NSW drivers’ licence.

Desirable Criteria

  • Ability to facilitate educational workshops and develop quality educational resources.
  • An understanding of the primary health care environment and experience in communicating with health care professionals and consumers within CESPHN boundaries.
