About the role
The Care Coordinator is responsible for the delivery of best practice, home based programs and services to Remedy clients Australia wide. Home-based care includes the provision of Rehab at Home and Hospital Care at Home programs and nursing, allied health and homecare services.
The care coordinator is an office-based role acting as a central point of contact for clients; implementing, monitoring and modifying in-home care plans to match the need of clients and manage risk. You will be based in a dynamic office environment and be part of a warm, bold and honest culture.

Skills and experience
To be successful in this role you will have:
- Registered Nurse, Division 1 qualification, with minimum two years of acute, sub-acute or community nursing
- Previous experience working in a multidisciplinary collaborative team
- Extensive knowledge of hospital discharge planning, and resources available to clients in the community, with demonstrated breadth and depth of clinical knowledge
- Knowledge of community funding streams including private health insurance, Aged Care and DVA.
- Well-developed computer skills
