Our Metropolitan Community Services team are now seeking Service Planners (1 and 0.6) to join the team on a fixed term full-time basis (period ending June 2019).
Reporting to the Service Planning Support Manager, the Service Planners will be responsible for coordinating services tailored to suit the individual client needs, in accordance with the service agreement. This includes liaising with external agencies, conducting effective assessments and reviews of client needs, planning, and coordination and case management.
To be successful in this role you will demonstrate excellent time management skills, attention to detail and a conscientious work ethic. You will also possess:
/ An understanding of community resources and supports as well as excellent verbal and written communication skills,
/ A strong theoretical approach to working with ageing population and demonstrated practical application of this approach,
/ Ability to maintain data collection and outcome measurement systems,
/ Diploma in Community Services or equivalent work experience.
