Support Coordinator

Part Time  - 30.4 hours per week

Base Location:  Hobart

Bring your sound experience, positive attitude and empathy to this very rewarding and unique role assisting the people we support.  We are looking for an NDIS-experienced person that will strive to deliver the best possible support coordination and community participation.  We work to enhance lifestyles and develop the independence of people we work with so that they can achieve to their full potential.  The role will require occasional advocacy for them as you help them to pursue their dreams and independence.

Support Coordinators facilitate the identification of goals and development of action plans for people with a disability.  We support people with a wide range of ages and disabilities - from tweenagers, to older people with NDIS Support Coordination in their plans. Working with you and your personal choices, the Support Coordinator develops a practical support plan, organises services and supports as well as monitors the services to ensure they’re of high quality and are meeting or exceeding your expectations.

This position may require some out of business hours with flexibility in working hours to manage a health life / work balance.

What we need from you

  • Excellent organisational and time management skills
  • Exceptional writing and communication skills
  • Demonstrated ability to listen and learn quickly
  • Good understanding of ethics
  • Valid Australian Driver's Licence, reliable and registered vehicle and car insurance
  • Current First Aid and CPR certificate
  • Current Police Check or willingness to obtain
  • ACT Working with vulnerable people clearance
  • Commitment to the rights of people with disabilities
  • A good knowledge and experience in Microsoft Office Suite and ability to learn database entries.
