Based in Brisbane, Support Workers will support individuals with a severe intellectual disability and/or mental health support needs, possibly challenging or complex behaviours generally between the hours of 6am - 8pm, Monday to Sunday. There will also be shifts outside of Life Stream's "Normal Trading Hours" - including sleepover shifts.

Key Responsibilities:

• Liaise with key stakeholders to ensure needs are met, activities are relevant and individuals supported are engaged and happy.
• Completion of administration tasks on time and correctly including daily program reports, monthly calendars and file notes.
• Communication with supervisors via email, file notes, program reports and phone as required to ensure disclosure, information sharing and seamless transition from one Support Worker to another.
• Provision of personal care if required.
• Participation in staff meetings, training, team meetings and supervision meetings as required.

Requirements:

• Must use your personal vehicle (4 door) for transporting of individuals during programs (kilometres paid when providing direct support to member only). Please Note - you may be asked to drive a 12 seater vehicle to support participants with higher support needs (standard card licence acceptable)
• Must be contactable by mobile phone during work hours.
• Must have a Smart Phone or Tablet (with internet capability) to comply with submission of timesheets and reporting requirements.
• Must have a current First Aid and CPR certificate.
• Expect to commence work upon arrival at participant's home and cease work upon departing participant's home at the conclusion of the program.
• Commitment to the values of dignity, respect, inclusion, empowerment, confidentiality and strength based practice.
• Ability to think creatively and use initiative while working within the service guidelines and professional boundaries.
• Ability to work independently and as part of a multidisciplinary team.
• Strong written and computer skills for the completion of program reports, correspondence and other documentation.
• Ability to develop positive relationships with a broad range of internal and external stakeholders.
• Established problem solving and conflict management skills.

To be maintained during your employment:

• Current driver's licence and a roadworthy vehicle with roadside assistance.
• Current first aid and CPR certificate.
• Willingness and ability to obtain a Public Safety Blue card and a Department of Communities Prescribed Notice card (or exemption card) or to have existing cards verified.

Life Stream Austraila's office is located at Stones Corner. Support Workers are expected to support participants that reside in all regions of Brisbane - however are predominantly  looking at filling support hours of participants who are located on the North Side of Brisbane (approximately 15 - 20 hours hours per week - depending upon availability). Life Stream Australia is also looking at filling support hours of participants who are located on the South Side of Brisbane (approximately 10 hours per week - depending upon availability).
