- Provide complex case management for Clients to remain independent in their Home

- Overseeing the quality management framework to ensure standards are met
- Supervision and support of staff reporting to this position
Working in the Sunshine Coast Region, you will be Coordinating the planning and implementation of a Model of Care which provides customer focused quality care services.
What do we need from you?
- Demonstrated experience in Aged Care package management
- Current CPR and First Aid Certificates
- Current open Australian Drivers Licence
- Reliable registered motor vehicle, with comprehensive car insurance
- A satisfactory National Police check obtained within the past 3 years
Nice to have
- An understanding of relevant Queensland funding bodies
- An understanding of Aged Care Reforms
- Experience managing referrals
- Experience in Support roles directly overseeing staff
