We are currently searching for an experienced Full Time Non-Clinical HomeCare Planner with relevant understanding and experience in the delivery of Aged Care packages or community based services. This role includes but is not limited to –

  • Complex Case management for high level Support Packages
  • Provision of customer focused quality care aiming to improve lifestyles
  • Overseeing the quality management framework to ensure standards are met
  • Supervision and support of staff reporting to this position

Working in the Sunshine Coast Region, you will be Coordinating the planning and implementation of a Model of Care which provides customer focused quality care services.

 What do we need from you?

  • Demonstrated experience in Aged Care package management
  • Current CPR and First Aid Certificates
  • Current open Australian Drivers Licence
  • Reliable registered motor vehicle, with comprehensive car insurance
  • A satisfactory National Police check obtained within the past 3 years

Nice to have

  • An understanding of relevant Queensland funding bodies
  • An understanding of Aged Care Reforms
  • Experience managing referrals
  • Experience in Support roles directly overseeing staff
