About the Role:

The Community and Youth Coordinator will be responsible for overseeing the expansion and delivery of the GP's in Schools program in local secondary schools.

In addition to this, the role will work with local providers to further identify, develop and implement targeted health promotion and prevention activities and campaigns that address the youth at risk.

  • Deliver high quality, evidence based youth activities in the SNPHN region
  • Expansion of GPs in Schools program across Private and Public Secondary Schools in the region.
  • Evidence of well-developed strategic partnerships and collaborations with other key providers to ensure maximum reach and depth.

Key Responsibilities:

Reporting to the Community and Partnerships Manager, the Community and Youth Coordinator will have key accountabilities in the following areas:

  • Formulate realistic and measurable program plan with achievable goals
  • Incorporate suitable evaluation
  • Actively build partnerships which enhance youth health program delivery
  • Provide timely, high quality reports and other written communication for a variety of audiences
  • Collaborate with other providers in the region to provide strategic, collaborative high quality and appropriate activities to youth.
  • Represent SNPHN at relevant forums
  • Active Support of the wider Commissioning and Partnerships Business Unit

To be successful in this role you will need to meet the following criteria: 

Essential

  • Health or health program delivery
  • Experience formulating measurable goals and objectives to inform a health promotion program
  • Experience in establishing successful partnerships with relevant organisations and facilitating collaborative action
  • Demonstrated ability and experience in the design, delivery and evaluation of community based education projects which utilise evidence based strategies
  • Demonstrated ability to write reports and other written communication for a variety of audiences
  • Experience working as part of a multi-disciplinary team across multiple projects with the ability to apply effective interpersonal skills
  • Understanding of health promotion theory and practical application.

Desirable

  • Tertiary qualifications in health promotion, public health, social science (with a health promotion major), health education or related
    field.
  • Qualifications in a health related field e.g. Health Science, Nurse etc
  • Experience in the Australian health care sector.
  • Knowledge of Australian Health Care Reform and key stakeholder

Remuneration

A base salary commensurate with the not-for-profit sector and your skills and experience will be provided, and as a not-for-profit, attractive salary packaging options are available.

Special conditions

You will require some flexibility in your work schedule to meet business needs which may require out of hours work. 

We will be relocating to Chatswood later in 2018.

Successful candidates will be required to consent to a National Criminal Record, Reference and potentially Working with Children Checks if a requirement of the role.
