About the Role

  • Part Time positions available
  • Maximum Term Contract until 2020
  • 7 Day Rotating Roster (some sleepover shifts required)

 

Towards Home: Supportive Pathways to Permanent Housing has been developed to address the growing number of rough sleepers in the Melbourne CBD. This program provides 24/7 high intensity flexible support to these consumers.

As a Community Rehabilitation and Support Worker, you will provide assertive outreach and engagement, supporting consumers who have experienced primary homelessness with a focus on immediate needs regarding mental health, housing, substance use, chronic illness, financial disadvantage, recovery and social inclusion.

 

Core duties

  • Provide practical support to improve daily living skills
  • Manage the Towards Home transitional accommodation
  • Working with community partners
  • Plan, facilitate and evaluate group rehabilitation programs
  • Engage consumers and develop trusting and professional relationships
  • Work closely with clinical case managers in order to deliver the best possible comprehensive service to consumers

 

What will you bring to the role?

  • Experience in working with people with a psychiatric disability
  • Experience in working with homelessness
  • Ability and willingness to work effectively as part of a team, to meet program goals and objectives
  • Knowledge of complex mental health and drug and alcohol issues.
  • Highly developed written and verbal communication skills
  • Be a team player
  • Knowledge of the recovery model

  • Current Working with Children Check
  • Australian Working Rights
  • Drivers licence
  • Police check (refer to PD) – disclosable outcomes considered
