The Yudhilidin Mental Health Care Coordinator is an exciting role that would interest an experienced care coordinator with a passion for the well-being of Aboriginal people with a mental illness.
The position will be focused on providing a service in the Greater Taree and Great Lakes area with regular travel to Newcastle. The position will work as part of an integrated team to improve access to treatment and support for Aboriginal and Torres Strait Islander people with a severe mental illness. The Care Coordinator will involve proactive engagement with individuals, their families and carers and enhance the management of their illness in the primary care setting as well as assisting increased self-management.
This position is fixed-term until 30 June 2019.
