Care Coordinator - Aged Care

Client
We have been approached by a leading welfare organisation that provide a range of aged-care services across NSW to assist in the recruitment of Care Coordinator on a full time basis initially for 2-3 months with the possibility of the role being extended/made permanent. The role is based in the Inner West/Easter suburbs and will cover clients in those areas and also South Sydney.

Role
As the successful candidate your role will be to manage clients that receive CHSP funding, ensuring that they have choice and flexibility in the way that the care and support is provided. Services may include personal or domestic assistance, food and nursing services, medication supervision, transport, home maintenance, and mobility equipment, and your role will be to arrange the provision of these services

Criteria
As the successful candidate, you will have:

• Relevant tertiary qualifications
• Experience working in a case management role with in the aged care sector 
• An understanding of Home Care Packages and Consumer Directed Care
• Driving License and access to own vhicle

Further Info
In return, you will be offered a pro-rata salary in the region of $67-69k + Super + the benefits of salary packaging. You will also be compensated for any travel using your own vehicle.
