About the role

We are seeking a motivated and experienced professional to join our exceptional team, as a NDIS Support Facilitator. The NDIS Support Facilitator is responsible for facilitating The Disability Trust's School Leaver Employment (SLES) program. The role involves ensuring the provision of high quality services that are person centred and based on the individual needs and choices of the participant and their family.

What we are looking for:

  • Knowledge of school to work transition particularly for young people with a disability.
  • Experience and skills that demonstrates your ability to deliver positive support service options to people with a disability
  • Ability to enhance, facilitate and deliver the development and ongoing delivery of The Disability Trust’s School Leaver Employment Supports (SLES) program.
  • Promote The Disability Trust’s services to potential participants, through liaison with schools, transition centres, job network providers, families, participants/clients, other disability services and Government agencies.
  • A comprehensive understanding and working knowledge of the NDIS, including NDIS School Leaver Employment Support and School to Work Transition.
  • Experience in planning and scheduling of activities for people with a disability which include staff rosters
  • Understanding and commitment to participant centred service delivery.
  • A demonstrated understanding and the application of Work Health and Safety.
  • Excellent communication and proven computer skills. 
  • Drivers licence and access to a reliable/registered and insured vehicle.
  • Working with Children Check.
  • Cleared Criminal Record Check obtained within 12 months.
