The Goodwood Park Healthcare Group is a progressive rehabilitation-based organisation and a leading provider of support services for people who have a mental health illness, traumatic brain injury or spinal injury, or in some cases, a dual diagnosis.

We run a variety of services, but specific to this role is our Supported Living Service. This is a community based service that plays an integral role in supporting people to maintain and increase their independence, regain old and develop new skills, integrate into the community and improve their quality of life. Our contract is with ACC so 90% of our clients are suffering the after effects of a serious injury.

The Supported Living Assessor works to provide high quality assessment and review of clients’ needs and abilities, with which they then develop supported living plans and strategies that best enable and promote independence and a higher quality of life for our clients.

Tasks of a Supported Living Assessor include the following:

  • Complete client assessments and Supported Living plans in collaboration with ACC, the SLS Team Leader, the client, their significant others and service providers.
  • Review Supported Living Plans and complete initial outcome reporting, and ensure compliance with contract requirements and ACC.
  • Attend case conferences with ACC, the client, their significant others and service providers.
  • Provide support to Support Workers in relation to Supported Living Plans and their service delivery.
  • Take responsibility for clients care and develop and recommend interventions and treatments in consultation with the client, their significant others and service providers.
  • Develop therapeutic relationships with clients and facilitate discussions between clients, ACC, their significant others and service providers.
  • In all areas of work ensure legal compliance with all relevant health and disability legislation and the Organisations policies and procedures.
  • Actively pursue your own professional development as provided by the organisation.

The successful candidate must have the following skills, experience and knowledge:

  • Experience completing health assessments and client support plans in collaboration with clients, families/whanau and service providers
  • Minimum four years experience working in the health/disability field
  • An in-depth knowledge of local area and community resources
  • Experience working with people with a diverse range of cultural and lifestyle backgrounds
  • Minimum of a level 7 NZ qualification in the health/disability field
  • Proven report writing skills, computer literacy, and Word, Excel and Outlook skills
  • Eligible to work in NZ and have a full, clean NZ driving license

This position is a full time, Monday to Friday role, however, flexibility of hours across the working week is available. The Goodwood Park Healthcare Group has a full staff training program, and successful applicants will be supported to access further training and career opportunities.

Our office is based in Kumeu; however, this role will include travel across the wider Auckland region, with the use of a commute vehicle.
