If you are an experienced coach or facilitator and have a passion for helping people make positive changes to the way they lead their lives, then this Health Coach role in the Taupo Extended Care team is for you.

We are a primary care innovator. We're known for doing things a little differently to bring about change in the system - and that is where you fit in.
This new role will work collaboratively with general practice to support their high care needs patients to meet their long-term health goals. The role has a coaching and coordination focus to support client goals. These goals are often agreed with the general practice team working in the Health Care Home model.
The Health Care Home is a primary health care model that gives patients more control, the practice team an environment where quality of care and innovation can flourish and enables truly proactive, coordinated care for those that need it most.
This role will also support packages of care and while this might sound a lot, you won't be doing it all on your own. You will be part of a great team including likeminded health professionals such as our Exercise Consultant, Nurse Practitioner, Community Health Worker and Dietician.
The Taupo locality is one where innovation thrives based on the Health Care Home model and integration of primary care, general practice and community services. This is a great opportunity for you to further develop your skills and contribute to innovative services that can make a difference.
