The Role

This community-based Social Worker role is an extension of a historical service, supporting individuals and whanau enrolled with Te Whareora O Tikipunga - a general practice and social service provider. As a registered Social Worker you will support the identification of health and social service need among our enrolled population, and work constructively and collaboratively with other health professionals to find solutions tailored to each person and whanau. You have the chance to work closely with the general practice team, using your expertise to guide the development of innovative approaches to addressing the social determinants of health. 

Whilst you will work with a broad range of demographics, your priority will be engaging with our enrolled rangatahi. 

Building close working relationships with primary and secondary care clinicians and community groups is a key component of this role, and you will support the development of care plans that reflect the needs and desires of the person, their family and whanau, and that can be shared across other health professionals.

Based in Whangarei, the majority of our community members are local, although you may be required to travel across Whangarei. This role is part-time (16hrs/wk).

About You

As a passionate and inspirational Social Worker, you are keen to progress your career with an opportunity to truly shape the lives people lead. You will have at least two years post-graduate experience, and can demonstrate your expertise and passion for working with rangatahi in the community, and actively supporting their whanau environment. You will have a passion to contribute to the development of a high needs community, in conjunction with a team of health professionals based within a Neighbourhood Health Home. 

About Us

Te Whareora O Tikipunga is an established provider of health and social services in Whangarei, and through our sister hub in the Bay of Islands. We are passionate about improving the health and wellbeing of all of our community, especially the most vulnerable. Our services continue to grow as we become a Neighbourhood Health Home, and currently include general practice, physiotherapy, plunket clinics, school-based health services, social work, and clinical pharmacy. 

Based at our Healthcare Home in Tikipunga, Whangarei, this role seeks to grow a historical service, and we look forward to sharing the Northland lifestyle with you.
