We currently have a fixed-term, full time opportunity for a highly motivated and experienced Diabetes Educator committed to delivering high quality, culturally appropriate and evidence-based diabetes education.
This is a great opportunity to become involved with an organisation that is dedicated to improving health outcomes at the community level. Your experience in diabetes education service provision will be highly valued, as will your passion, energy and enthusiasm towards Aboriginal health, rural health and preventative health. Although you will be based in Karratha you will be expected to undertake outreach to towns and communities in the region.
Selection Criteria
Essential
- Current AHPRA registration, Registered Nurse
- Demonstrated experience in providing diabetes education and promoting patient self-management within hospital, primary care and/or community based setting
- Demonstrated experience and understanding of pharmacology of diabetes medications including problem solving and guidance
- Ability to provide holistic quality care in Diabetes Education
- Experience in working with Aboriginal and Torres Strait Islander people with an understanding of and appreciation for Aboriginal culture
- A strong background in chronic disease management and patient assessment
- Demonstrated and well developed interpersonal and communication skills with the demonstrated ability to develop and maintain productive relationships with people with diabetes, their families/carers and work within a multidisciplinary team
- Demonstrated effective organisational skills, with the ability to problem solve and apply principles of clinical risk management and quality improvement to the clinical setting
- Competence with clinical software programs and Microsoft Office suite of applications
- Knowledge of legislative and regulatory requirements in Occupational Health and Safety, Equal opportunity and Disability Services and how these impact on employment, people management and service delivery processes
- Current Police Clearance, Working with Children Check, Senior First Aid and Driver's Licence
Desirable
- Credentialed Diabetes Educator (CDE) with ADEA or working towards
- Experience in rural and remote health settings
- Ability to travel as part of a multidisciplinary outreach team
- Knowledge and understanding of the local health system, including referral pathways, community resources and services
