About the role
We are currently seeking an Aged Care and Palliative Care Clinical Nurse Consultant to join the Aged Care Plus clinical leadership team in this brand new role. The key responsibilities of the role are to support the Manager – Clinical Governance through establishing, directing and managing clinical care to promote resident / customer safety and clinical excellence within The Salvation Army. The Aged Care & Palliative Care CNC provides high level expert advice to the organisation on all clinical issues and works collaboratively with key stakeholders to analyse, maintain and improve resident/ customer safety and clinical quality systems across the organisation.
This is a permanent full time position based at our modern hedquarters in Redfern with travel to Aged Care Plus Centres as required.
The main responsibilities of this role include, but are not limited to:
- Provide clinical expertise to all resident / customer needs to optimize health outcomes and support choice and decision making
- Ensure all resident / customer care is aligned to care planning and optimal health and / or choice outcomes
- Clinical leadership of all complex care needs and close monitoring
- Implement and support 24/7 clinical decision making and hospital avoidance strategies
- Providing clinical advice and direction into clinical governance processes and support Centres through the provision of complex clinical management support and case management where appropriate for complex clinical and health needs of residents/customers
- Ensure effective identification, investigation, management, monitoring and reporting of clinical safety risks and adverse resident / customer events
- Providing clinical analysis and advice for complex clinical complaints and/or issues management and liaise with the relevant key stakeholders
- Provide education and clinical supervision to ensure capability to staff and consistent best practice clinical care to residents and customers
- Actively seek research and data analysis opportunities to appraise emerging trends or evaluate current practices in order to identify initiatives with the potential to improve care and service delivery or create business opportunities, analyse costs and benefits and further explore opportunities
- Provide evidenced based, standardised and timely resources for staff to improve care provision and support decision making
About You
To be considered for this important role, you must have:
- A commitment to the Christian faith and mission of The Salvation Army. An ability to offer pastoral assistance, to offer prayer and to explain the Christian Gospel
- Registration as a Registered Nurse without restrictions in accordance with AHPRA requirements
- Post graduate qualifications in aged care, palliative care and / or relevant field
- Minimum 5 years post basic experience preferably in Aged Care
- Significant Expertise / Credentialing in Aged Care and / or Palliative Care
- Conceptual and innovative problem-solving skills for managing conflicting priorities and developing timely, successful recommendations and advice on policies, strategies and solutions
- Strong interpersonal, communication, influencing and motivational skills which demonstrate a capacity to build and lead
- Demonstrated capacity to lead clinicians and to initiate and achieve significant change and improvement in clinical governance
- Demonstrated high level written, oral and interpersonal communication skills
- Experience in management of stakeholder relations with key stakeholders
- Current Drivers Licence
- Willingness and ability to take on-call requirements on a rotational basis
