As a Case manager your responsibilities will include:
- Visiting clients, building strong relationships and maximising their care packages
- Be a Brand Ambassador for the company
- Liaise with stakeholders, employees, service providers on behalf of clients
- Complete annual reviews of all clients and revising their plans and budgets
- Continue to investigate new ways to add value and build client enablement
- Minimum of two years in a similar position as a Case Manager / Client Advisor
- Previous experience in the health industry or as a social worker highly regarded
- Degree qualified
- The ability to work autonomously
- Self-sufficient and driven to deliver best outcomes
