Home Support Services (HSS) has been providing care nationally to patients in the comfort and security of their own home since 1989. We are passionate about providing holistic and well managed care which supports our patients to be cared for in their own home. 

Duties

• Conduct postnatal assessment planning and evaluation for clients within the postnatal care program
• Some delivery of anti natal care services
• Provide postnatal care and support to meet the needs of clients and their families
• Liaise with stakeholders, GP's, consultants and other health professionals to provide high quality care
• Facilitate multidisciplinary team meetings
• Mentor, support and educate team members
• Manage daily operations
• Assist with report requirements
• Assist with program strategic development

Skills & Experience  

• Registered Midwife with minimum 2 years experience
• Experience leading a team 
• Intermediate Computer skills
• “Everything for the Patient” ethos

Required

• Current AHPRA Registration
• Drivers Licence
