Our client is a well-known not for profit specialises in aged care, providing home care services and solutions. They are an expert within the industry with over twenty years’ experience.

As a Case manager your responsibilities will include: 
  • Visiting clients, building strong relationships and maximising their care packages
  • Be a Brand Ambassador for the company
  • Liaise with stakeholders, employees, service providers on behalf of clients
  • Complete annual reviews of all clients and revising their plans and budgets
  • Continue to investigate new ways to add value and build client enablement
To be successful as a Client Advisor/Case Manager you will need: 
  • Minimum of two years in a similar position as a Case Manager / Client Advisor
  • Previous experience in the health industry or as a social worker highly regarded
  • Degree qualified
  • The ability to work autonomously
  • Self-sufficient and driven to deliver best outcomes
