Seeking an experienced professional in the disability sector to assess and plan for clients in the community.

Your new company
Working for the local government in far north east Melbourne, this department of the local government specify in community services based around disability and elderly support.

Your new role
As the Assessment officer you will be expected to work between the main office and in the community on a full time basis for a short period contract.
Your responsibilities and duties will consist of:

  • Undertake face to face assessments to vulnerable residents/clients, requesting and/or requiring access a range of home and community support services, allied health and community nursing services.
  • Use a wellness and reablement approach to develop support plans which identify client and/or carer needs and capacities and focus on building existing strengths and interests to promote independence.
  • Make appropriate referrals to other services within agreed time frames.
  • Review and evaluate support plans, ensuring changing needs are responded to in a timely manner.
  • Adhere to protocols and agreements between Council and other relevant agencies, organisations and individuals accessing assessments and care management services.
  • Actively participate in communication strategies and foster a collaborative approach across the Community Wellbeing Unit to ensure service integration and coordination of activity and focus.
  • Ensure that the comprehensive system of client records and statistical data for accountability to Council and funding bodies is maintained by the accurate and timely input of data in both My Aged Care and Councils internal client management systems.
  • Work cooperatively within the Community Wellbeing team to support the provision of quality customer support at all times.
  • Participate in weekly Care Coordination Meetings as relevant.


What you'll need to succeed
To be successful in this position you will:

  • Have demonstrated knowledge and understanding of the Home and Community Care Program for Younger People, Wellness/Reablement strategies, the Commonwealth Home Support Program and the range of Aged Care services and how these are accessed.
  • Display a thorough understanding of issues around the needs of older persons, persons with a disability and their carer.
  • Highly developed skills to work within the My Aged Care web based portal and client management systems are required.
  • Experience in conducting assessments of clients needs, particularly for older persons or people with disabilities, goal setting and developing care plans to meet these needs.
  • Ability to provide a high level of verbal and written communication skills.
  • Strong negotiation and influencing skills.
  • Proven ability to influence and gain cooperation of key stakeholders.
  • Ability to manage personal workload and program requirements within timelines, available resources and overall Council policies.
  • Ability to coordinate services and to work as part of a team.
  • Ability to meet performance targets.
  • Acts in accordance with the team member role and responsibilities of the How We Work Capability Framework.

  • A tertiary qualification in assessment, social work, welfare work, nursing and/or allied health is essential.
  • Substantial experience, knowledge and understanding of the Home and Community Care Program and Commonwealth Home Support and assessment policy development, planning and service provision within local government or related environment is highly desirable.
