We have an opportunity for an experienced Clinical Care Coordinator to work closely with and support the Director of Nursing. The organisation promotes person centred care with the goal of improving the ‘living in care’ experience of residents of this 56-bed home. I
This position is suited to an individual with motivation and drive. The organisation believes in a supported clinical model of care which in turn provides the highest quality care to residents.
Clinical Care Coordinator Profile:
- Current AHPRA registration
- 4-5 years of experience as Clinical Care Coordinator
- Strong leaderships skills
- Strong Clinical skills
- A dynamic personality, who will motivate the team
- Excellent communications skills – written and verbal
- Current National Police Check
- Working Knowledge of Aged Care Legislation and Accreditation, Policies, ACFI, ACRIS, OH&S legislation
- Experience with change management
- An ability to lead by example and motivate staff
- A down to earth, positive and mature approach
