The Organisation:
Bridges Health and Community Care is a leading provider of Mental Health and Drug & Alcohol Services across Bundaberg, Fraser Coast and North Burnett, operating in the region since 1997. We now provide services up north to Gladstone, west to Cherbourg and Kingaroy and south to Maroochydore and Gympie. Bridges offers a variety of interventions and supports for all age groups including psychological services, outreach, counselling, education and training, healthy lifestyle and life skills programs and psychosocial rehabilitation. About the opportunity:
Bridges Health Community Care is seeking an experienced Social worker to join our multidisciplinary team in our Mental Health and Disabliity Program.
Bridges has a strong client base with consistent referrals from our hospital and Health Service, general practitioners, Corrective Services and many other providers.
We work with clients with a range of issues from complex PTSD, Addictions, trauma, autism and both psychotic and non-psychotic disorders.
We are looking for an exceptional practitioner who wishes to part of a community-based team that can provide an integrated response utilising Bridges other programs and staff to ensure that clients receive a range of interventions depending on their needs.
To be successful you must have additional training and skills in working with people with mental illness from a recovery practice framework, provide genuine person-centred care that empowers individuals to reach their full potential and sound counselling and case management skills.
Key Selection Criteria
Essential:
- Tertiary qualifications in Social work or equivalent an experience in working with people with moderate to severe mental illness.
- Minimum 5 years post graduate experience in providing professional social work services
- Demonstrated ability to maintain high quality case notes and client records
- Excellent organisational and time management skills and the ability to operate autonomously
- Commitment to evidence-based practice, continuous quality improvement, training, and professional development
- Strong IT and computer skills including use of databases
- Satisfactory Police and Working with Children Clearances or willingness to obtain;
- Current driver's licence;
Desired:
- Knowledge and understanding of the National Disability Insurance Scheme (NDIS) and what this means for clients
