PHNs have been established by the federal government with the key objectives of increasing the effectiveness and efficiency of medical services for patients, particularly those at risk of poor health outcomes and improving coordination of care to ensure patients receive the right care, in the right place, at the right time.

 The role

Your purpose involves the implementation of key tasks and strategies within various health priority areas, in order to facilitate specific programs/projects approved by the Commonwealth.

The overarching key duties include:-  

  • Assist to design, develop and operationalise successfully approved programs from concept phase to implementation phase based on evidence based practice. 
  • Develop and monitor the procurement, contract management and service delivery of approved programs, including service performance monitoring and evaluation processes in accordance with contractual requirements
  • Identify, consult and manage relationships with key stakeholders involved in potential and or existing, approved initiatives, including communication and networking arrangements and the establishment and maintenance of advisory committees where required.
  • Contribute to the development and implementation of regional planning in accordance with defined contractual requirements
  • Maintain and manage operational plans, contracts and or service agreements with health providers and where applicable contribute to the reporting of contractual obligations and the strategic and operational plans of our PHN   
  • Support formalised evaluation of approved projects

Goals

  • Initiatives and programs that support improvement of health services will be scoped developed, implemented and costed for alignment with the expected strategic objectives of the organisation. 
  • Key stakeholder engagement activities to assist in the co-design of health improvement activities will be undertaken in a professional manner at all times.
  • Successfully identified initiatives and programs will be incorporated into our strategic plan and operationalised in accordance with contractual requirements.
  • Approved initiatives will be implemented, coordinated and maintained within budget and on time in accordance with contractual guidelines 
  • Implementation of assigned initiatives will be underpinned by a Performance Framework with an emphasis on continuous improvement and evaluation of outcomes.
  • Ensure our PHN is a key stakeholder in the successful implementation of projects that support positive outcomes towards improving the patient journey.

About you

The successful candidate will meet the following selection criteria:

  • Tertiary qualifications 
  • Demonstrated knowledge of the principles underpinning the population health approach and social model of health.
  • Experience in project planning, scoping, development and  evaluation
  • Demonstrated ability to communicate and liaise effectively with a wide range of stakeholders, including GPs, researchers, service managers and community representatives.
  • Demonstrated knowledge of the primary health system in Australia, including its interaction with the acute care sector, as well as the challenges and opportunities.
