We are currently seeking applications for Senior Registered Nurse - Community Coordinator to join our small friendly Community Care Team at Red Hill. The purpose of this position is to coordinate the daily operations of the Community Home Care Packages and oversee client’s health across QLD, NSW and VIC. As a Coordinator, you will play a pivotal role in ensuring a high standard of care is maintained at all times, manage a small friendly and dedicate team of Home Care employees and provide Nursing assistance within the Community.
The successful applicant will demonstrate:
- A prior experience in a clinical or similar environment;
- A Nursing Registration with the Australian Health Practitioner Regulation Agency (mandatory);
- Minimum three (3) years’ experience in a leadership role;
- The ability to provide advanced level of case management and coordination of service provision to clients;
- The ability to lead team of coordinators and work collaboratively with the Community Care Manager for the effective management of the Community Care programs;
- A shared responsibility for the achievement and maintenance of quality improvement activities, including the Community Service quality reporting Management and monitoring of the Community Home Care Employees within the workplace including the completion of work performance appraisals;
- The ability to attend to clients in a Registered Nurse capacity for those client whom have Nursing care as part of the community package;
- The ability to comply with legislative and organisational reporting requirements; and
- A current valid Open Driver's Licence.
