About the Baker Institute

Baker Heart and Diabetes Institute is an independent, internationally renowned medical research facility.  Our work extends from the laboratory to wide-scale community studies with a focus on diagnosis, prevention and treatment of diabetes and cardiovascular disease.

Since 2011, the Baker Institute Central Australia has been providing diabetes outreach services to Central Australian remote Aboriginal communities. The Baker Institute’s diabetes outreach services are based on a multidisciplinary model of care which pairs Diabetes Nurse Educators located in Alice Springs with visiting endocrinologists. Outside of specialist visits, Diabetes Nurse Educators work independently in a remote setting, which requires a high level of experience and good clinical judgement.

We seek a Diabetes Nurse Educator to join our team in Alice Springs.

About the Role

Diabetes Nurse Educators (DNEs) are critical in establishing continuity with community health clinics, staff and patients; these professional relationships are a foundation for ongoing success of the service. The DNEs provide patient consultation and staff education prior to the specialist visit, support specialists during their visit, and support patient follow up (both in person and remotely) after the visit. Outreach specialists benefit from the relationships established through the DNE.

Duties

In this role you will be expected to:

  • Coordinate a consistent approach to diabetes management, care and education with patients and clinics in remote communities.
  • Provide the highest quality diabetes education service to individuals with diabetes, their family members and other supports.
  • Support specialist through clinical assistance and client education.
  • Carry out assessments, planning, clear documentation, delivery and evaluation of comprehensive education programs for clients with type 1, type 2, gestational diabetes and pre-diabetes.
  • Maintain accurate and timely patient records and program reports.
  • Carry out appropriate data entry for internal and external reporting.
  • Undertake appropriate health promotion activities.

Skills and Experience

You will have:

  • Registered Nurse Division 1, holding current practising certificate with at least 3 years post-graduate experience.
  • Credentialed with ADEA as a Diabetes Nurse Educator or working towards this status.
  • Experience in providing clinical care and education to individuals with diabetes.
  • Experience in and knowledge of health service delivery in Alice Springs and surrounding communities or similar remote locations, including in/with Aboriginal communities and Community Controlled Health Service providers.
  • Proven ability to work both independently and as part of an inter-disciplinary clinical team in a remote community setting.
  • Highly developed diabetes management knowledge and skills is desirable
  • Exceptional communication skills, including attention to detail and the ability to deal successfully with a broad spectrum of people including Aboriginal people living in remote communities.
  • Ability to communicate effectively within multidisciplinary team members, including nurse educator colleagues, specialists and leadership (clinical and project leads).
  • Excellent planning and organisational skills, including the ability to manage a range of tasks with conflicting priorities.
  • Current Driver’s license (manual and automatic) and experience driving independently in remote areas.
  • Ability to travel for long distances by 4WD and light aircraft.

The successful candidate will be required to present a valid NT Working with Children card or be willing to apply for one upon commencement.  The Baker Institute will provide the successful candidate a complete cultural awareness and 4WD training as a condition of employment.
